injection was given incorrectly at an angle which caused significant pain; some of the solution ran down the patient's arm, the doctor and nurse decided to give another injection in the other arm; some of the solution ran down the patient's arm, the doctor and nurse decided to give another injection in the other arm; acute headaches resulting in mri; significant pain causing the patient to scream out; this is a spontaneous report received from a contactable reporter(s) (consumer or other non hcp) from the regulatory agency (ra). the reporter is the patient. other case identifier(s): gb-mhra-vac-202202211728222440-pwyju (ra), gb-mhra-adr 26631177 (ra), gb-mhra-vac-202202211728222440-pwyju (ra). a 75 year-old female patient received bnt162b2 (comirnaty), administration date 03nov2021 (batch/lot number: unknown) at the age of 75 years as dose 3 (booster), single for covid-19 immunisation. the patient's relevant medical history was not reported. concomitant medication(s) included: lansoprazole. vaccination history included: covid-19 vaccine (dose 2 ,manufacturer unknown), for covid-19 immunization; covid-19 vaccine (dose 1 ,manufacturer unknown), for covid-19 immunization. the following information was reported: wrong technique in product usage process (medically significant) with onset 03nov2021, outcome "unknown", described as "injection was given incorrectly at an angle which caused significant pain"; incorrect dose administered (medically significant), overdose (medically significant) all with onset 03nov2021, outcome "unknown" and all described as "some of the solution ran down the patient's arm, the doctor and nurse decided to give another injection in the other arm"; headache (medically significant) with onset 03nov2021, outcome "not recovered", described as "acute headaches resulting in mri"; vaccination site pain (medically significant) with onset 03nov2021, outcome "unknown", described as "significant pain causing the patient to scream out". the patient underwent the following laboratory tests and procedures: magnetic resonance imaging: unknown results. therapeutic measures were taken as a result of headache. clinical course: reported as, the patient experienced constant and continuing acute headaches which started the day of the booster injection. the patient was waiting for results of mri investigation. it was reported that these reactions occurred as a result of a mistake made in the administration of the vaccine. the injection was given incorrectly at an angle which caused significant pain and caused her to scream out. as some of the solution ran down through her arm, the doctor and nurse decided to give her another injection in the other arm. the patient whilst resting she was brought paracetamol as a strong headache began and this was not stopped since. the doctor said not to worry as they had once by mistake given a patient a full bottle, 6 injections in one go and they were fine. the patient further stated that she was not fine and was still suffering much pain and cope taking medication which she does not wish to be taking. no follow-up attempts are possible; information about lot/batch number cannot be obtained. no further information is expected
