myocarditis; pericarditis; chest pain; dyspnoea; this is a spontaneous report from a contactable physician from the regulatory agency (ra). regulatory number: gb-mhra-webcovid-202202241700208630-hjpwr. other case identifier: gb-mhra-adr 26648275. an 18-year-old male patient received bnt162b2 (comirnaty), administration date 19feb2022 as dose 2, single for covid-19 immunisation. the patient's relevant medical history and concomitant medications were not reported. vaccination history included: covid-19 vaccine (dose 1, manufacturer unknown), for covid-19 immunisation. patient had not had symptoms associated with covid-19. covid-19 virus test was no - negative covid-19 test. patient had not tested positive for covid-19 since having the vaccine. patient was not enrolled in clinical trial. the following information was reported: myocarditis (hospitalization, medically significant) with onset 20feb2022, outcome "recovering", described as "myocarditis"; pericarditis (hospitalization, medically significant) with onset 20feb2022, outcome "recovering", described as "pericarditis"; chest pain (hospitalization, medically significant) with onset feb2022, outcome "unknown", described as "chest pain"; dyspnoea (hospitalization, medically significant) with onset feb2022, outcome "unknown", described as "dyspnoea". the patient was hospitalized for myocarditis, pericarditis, chest pain, dyspnoea (start date: feb2022, discharge date: feb2022, hospitalization duration: 2 days). the events "myocarditis", "pericarditis", "chest pain" and "dyspnoea" were evaluated at the physician office visit. patient was perfectly well, 24-36 hours post-dose, developed chest pains. the report was not related to possible blood clots or low platelet counts. the report was related to possible myocarditis and pericarditis. the patient admitted to hospital and stayed in hospital for 2 days. the patient was seen by a cardiologist and the diagnosis was myocarditis. the troponin was measured, this was taken on admission and until discharge. there was not presence of pericardial rub, or changes in heart sounds, no signs of heart failure or arrhythmia, no signs of a systemic disease that may be responsible for the presentation. the cardiac troponin (normal high less than 15) was abnormal, and the peak troponin value was 2000 in feb2022, and 1800 in feb2022, and 1500 in feb2022. in feb2022, patient also had normal chest x-ray, echocardiogram presented ef (ejection fraction) 54%. the chest computed tomography and coronary angiography were not carried out; cardiac mri would be carried out in future. no follow-up attempts are possible; information about lot/batch number cannot be obtained. no further information is expected
